Before you begin

You should only use this form if you are applying for the School Essentials Grant for a Looked After Child (LAC). Only one application can be made each school year, per Looked After Child. Applications for pupils who are not LAC will be disregarded.

You can use this form to apply for the School Essentials Grant for one LAC at a time. If you would like to apply for a grant for more children, please complete the form as many times as required.

You must have a bank account to receive a school uniform and equipment grant.
Looked After Child (LAC)
The pupil you are applying for must be in the care of either Denbighshire County Council or another local authority, and either:
- living with foster parents
- living in a residential children's home
- living in other residential settings like schools or secure units
- living with a parent, other relative or connected person and subject to a care order
